Parma Animal Shelter, Inc. Junior Volunteer Contract 

The Parma Animal Shelter, Inc. is run by a staff of Volunteers for the City of Parma. Our mission is to help lost, homeless and abandoned animals found in the City of Parma.  Our goal is to reunite lost animals with their humans and find good homes for those we shelter.

 All volunteers are expected to abide by the Rules of the Shelter’s Policies and Procedures.  In addition, junior volunteers are expected to abide by the rules in this contract.  Failure to follow any rules or directives may be grounds to be removed from the junior volunteer program.

 I, ___________________________________, hereby agree to follow the rules of the Parma Animal Shelter, Inc., my junior volunteer contract, and the rules as indicated by the Junior Volunteer Coordinator. 

1. Always sign the log at the beginning of my shift and note the time at the end.
2. Do not enter the kennel areas without adult supervision.
3. Never borrow supplies or equipment from the shelter.
4. Never be afraid to ask questions.
5. Always report any problems to my shift mentor.
6. Do not invite friends into the shelter unless they are accompanied by a parent.
7. Always arrive promptly at my assigned volunteer time.
8. If I cannot fulfill my commitment I will call the  Junior Volunteer Coordinator, giving as much notice as possible so a replacement might be found.

I understand the rules listed in this Junior Volunteer Contract and agree to obey all Policies and Procedures of the Shelter and the Junior Volunteer Coordinator.  I understand that I may be removed from the program if I do not follow the rules.

PARENTAL APPROVAL/RELEASE 

I, _________________________________, am the parent and/or guardian of the above Junior Applicant.  I herby approve my child’s entry into the Junior Volunteer Program, and consent to have my child/ward matched with a Parma Animal Shelter Sr. Volunteer.  I have reviewed the Junior Volunteer Contract and application with my child/ward, and also agree to be bound by the terms herein.  All information provided in the application is true and accurate to the best of my knowledge.

  I, for myself and my child, hereby waive and release any and all rights and claims for damages which I may have against the Parma Animal Shelter, Inc.,  City of Parma Animal Warden, any shelter volunteer.  I agree to hold the Parma Animal Shelter Inc., City of Parma Animal Warden,  and volunteers harmless from any and all liability, cost and expense arising out of my child/ward’s participation herein and any injury said child may receive.
