Parma Animal Shelter Patron Adopt Pets Album Application

Provisions:

ü      Patron must provide a non-returnable photo of the pet. 

ü      A $5 donation to Parma Animal Shelter, Inc. must accompany the application.  Mail to: Parma Animal Shelter, Inc., 6260 State Road, Parma, Ohio 44134 ATTENTION:  WEBMASTER  The photo will appear on the Adopt Pets page for a period of at least two weeks.

ü      The Patron must agree to inform the shelter when the animal is adopted.

This form must be signed and dated by the patron and a volunteer.

This Patron Adopt Pets Album is available on a limited basis, as space permits.  PAS Inc. reserves the right to limit the amount of applications accepted.  Patrons who have exhausted all other means to place their pets, i.e. advertising in the newspaper etc, will be considered for this program.
